Referrals: common questions
How do I refer a patient to your service?
Call 07575 298602 or email a discharge summary to enquiries@fabuloushomecare.co.uk. We respond the same working day (Monday to Friday) and arrange a nurse-led assessment. For a time-critical discharge, our phone line is answered day or night.
What clinical needs can you take on?
Nurse-led support for tracheostomy, PEG and enteral feeding, ventilator and respiratory needs, wound and pressure care, catheter and stoma care, and diabetes, plus condition-based support after stroke and for Parkinson’s, MS and brain injury. Carers are competency-checked per procedure, and clinical tasks such as wound dressings and catheter changes stay district-nurse-led where appropriate.
How quickly can care start?
Often within days, and faster when a discharge is time-critical. Tell us the situation and we will be honest about what we can put in place and when.
Is the care fundable through Continuing Healthcare?
Many complex packages are funded in full through NHS Continuing Healthcare, which is not means-tested. We provide Decision Support Tool / CHC supporting evidence, and we also work with case managers on case-managed and settlement-funded packages.
Which areas do you cover?
Croydon, Bromley and the surrounding towns, close to Croydon University Hospital and Princess Royal University Hospital. We take referrals from any team, no existing arrangement needed.
